In a perfect world, everyone who is unwell would approach a doctor, state all the symptoms, undergo tests, get the ailment diagnosed, start and finish a treatment with the same hospital and the same doctor. In that case, all the records associated with each patient would be available at the hospital or doctor's clinic, along with the doctor's observations on examining the patient, prescriptions and test results and so on. However this is not always the case.
